I upgraded his armor and gave him an axe. He dies when anything hits him. I checked his hit points and he has 1. 1 total. Did anything cause this, or is it fixable? I am only level 7 so i can restart if needed. Thank you

Check his armor for superior or major runes. Superior attribute runes subtract 75 health, and major runes subtract 35. Replace any you find with minor runes.

The only thing that springs to mind is

1: Do you have a supior rune or Major runes on him IE sword and if so do you have more then 1 of them as this will affect the durability of him this however does not apply to major vigor or superior vigor these are all fine just see if the take away his HP.

2: Koss does not have max armour his armour increases with levling

3: and depending on what weapons you have on him IE +30hp on them he should not be dying so quick

Koss is level 2 when you first get him.

He has 130 health.

Two superior runes and BAM, 1 health.
